SUD Authorization Schedule - Revised

Authorizations should be completed by Initial (for Outpatient H0001 and T1007; for Withdrawal Management 3 units of H0010 or H0012, S9976; for Residential 14 units of H0018 or H0019, S9976).  Re-Authorization requests should include ASAM and be individualized based on the client need.
