New Hire Notification Form
The NMRE requires a New Hire Notification Form for all staff providing services (prevention, treatment and recovery supports) or needing access to RECON.
For clinical staff, the New Hire Notification form must be approved by NMRE before provider staff can provide clinical services funded by the NMRE.
Completed forms should be submitted to email@example.com. Staff needing access to the RECON system must have had training on how to use the system and have their supervisor sign off as part of the New Hire notification process.
When staff leave employment, the NMRE requires notification of departure through firstname.lastname@example.org, including the last date of employments, to ensure confidentiality of all systems.
