Use of ICD - 10 Z Codes
Z codes are a special group of ICD 10 CM codes for reporting factors influencing health status and indicating contact with health services.
Z55 - Z65 (Socio-Economic Conditions) Persons with potential health hazards related to socioeconomic and psychosocial
circumstances)
Z77 - Z99 (Environmental Conditions)
Z80 - Z99 (Persons with potential health hazards related to family and personal history and certain conditions influencing health status)
Z codes listed above are the recommended codes to be used for Health Home tracking. Health Home Partners can use any z codes that are applicable to client conditions and that influence their health status.
Z codes should not be used as the primary diagnosis code.
A member of the care team identifying an enrolled beneficiary's social determinants of health should work with the other members of the care team and the diagnosing provider to ensure the appropriate z code is submitted on the Health Home encounter along with the OUD diagnosis; moreover, the care team must ensure that the social determinants of health identified are also documented in the beneficiary's care plan.
