Procedure for Identifying Client Needs on Treatment Needs Questionnaire
The Treatment Needs Questionnaire is required as part of the client screening and enrollment in the OHH program.
The form should be administered to clients, scored and reviewed by OHH staff to determine the level of treatment that is most appropriate for the individual.
If the treatment needs score indicates a level of service your agency is unable to provide then the client should be referred to the NMRE Access Center at 800-834-3393 for screening and discuss all MAT options.
