SUD Health Home (SUDHH) Resource Materials
MDHHS has developed resources for the Substance Use Disorder Health Home expansion serving individuals with a qualifying health diagnosis of Opioid Use Disorder, Alcohol Use Disorder or Stimulant Use Disorder. Individuals must also be at risk of developing a mental health condition, asthma, diabetes, heart disease, BMI over 25 or COPD.
