MDHHS hosted a Care Plan training in April 2026 to go over SMART Goals and Objectives and reviewed scenarios to determine what should be included in a beneficiary care plan.
The following materials are to be used as a guide for developing care plans for beneficiaries enrolled in health home services.
Please review the current MDHHS health home handbook which list out the care plan requirements.
